Sour, pounded. Both parts are transparent Thai — and the name still says nothing about green papaya, which is the one thing everybody thinks it means.
This is a trap for anyone who has learned that ส้ม means orange. Here it means sour, the older sense, and the same chain explains แกงส้ม, the sour curry, which is not orange-flavoured either. ตำ is a real method word you can hear from the pavement. What is left out matters at the mortar: fish sauce and dried shrimp are invisible in the name, and the useful instruction there is a number — ใส่พริกสองเม็ด — not an adjective. One more thing about the order. The rest of the family puts ตำ first — ตำไทย, ตำปู, ตำปูปลาร้า — and only the headword ส้มตำ puts it second. It is the odd one out in its own family, so do not lean on the word-order rule here.

What is in it Shredded green papaya, bird chillies (you choose how many), garlic, long beans, cherry tomatoes, peanuts, dried shrimp, palm sugar, lime, fish sauce. ปูปลาร้า versions add fermented crab and fish sauce.
Vegetarian and allergies ตำเจ (dtam jee) exists: no fish sauce, no dried shrimp, soy-based. At an Isaan stall, say it clearly and they know exactly what you mean.

The chilli-count order, the most useful number sentence in Thailand: ส้มตำไทย ใส่พริกสองเม็ดครับ (sôm-dtam thai, sài phrík sǎwng mét khráp), Thai-style, two chillies.
A2 Make it yours
Know the family: ตำไทย (sweet-sour, peanuts, safe), ตำปู (salted crab), ตำปูปลาร้า (fermented fish, the real Isaan one). Complete the set meal: ข้าวเหนียว (sticky rice) + ไก่ย่าง (grilled chicken).
B1 Talk about it
แซ่บ (sâep) is Isaan for delicious-with-fire and the ONLY right compliment here; แซ่บอีหลี (sâep ii-lǐi, truly sap) if you can take the heat. Discussing ปลาร้า (fermented fish) tolerance is a genuine friendship milestone.
C1 to native Own it
ส้มตำ is class-and-region politics on a plate: long sneered at as upcountry food, now on Michelin lists, and every Thai has a position on whether Bangkok som tam is a polite lie. มึงกินปลาร้าได้ปะ, "can you actually eat pla ra?", asked with the rude-intimate pronoun, is the moment you have stopped being a foreigner at the table.
